Accessing Health Information

Requesting Your Health Information

Your health information is available for your personal records or continuing medical care with other providers. You may find some information in your Patient Portal, but if it is not there or you are not set up for the portal, copies of your results or notes may be requested from our Health Information Management Department (HIM). For your convenience, please complete the Release of Information form and either mail or drop off the form. Patient Representative

We understand that you may have family or friends involved in your health care. For anyone to access your record, you have to give permission every time. This includes your spouse, adult child, parents of 18 and older patients. By what you allow access according to what you mark on the form, the Appointment of Representative gives those you designate the right to access your records, request copies, and discuss in depth your issues at any time. For your convenience, please complete the Appointment of Representative form and return to ACMH. Once this form has been scanned into your record, the designated representatives may have access.
