Skilled care is available to patients who no longer require an acute level of hospital care but are still not quite ready to leave and be on their own at home. The skilled care setting provides patients with nursing care, medication administration, individualized assessments and care plans, rehabilitation services, wound care and access to a physician along with other available hospital services.
Once the patient has progressed to meet their care plan goals, or no longer meets skilled care criteria, the hospital staff works with the patient and their family to determine discharge plans. Our goal is to return our patients to their optimum level of independence.
