Audubon County Memorial Hospital & Clinics Improves Chronic Care Management Services

Audubon County Memorial Hospital & Clinics is excited to announce that our new Chronic Care Management (CCM) service is being managed solely by the staff at Audubon County Memorial Hospital & Clinics.

The goal of this service is to help control chronic conditions, improve overall health, quality of life, and reduce the need for ER and hospital visits. Chronic Care Management (CCM) is a Medicare-created program designed for patients with two or more chronic conditions such as diabetes, hypertension, COPD, heart disease, Alzheimer’s disease, hyperlipidemia, glaucoma, congestive heart failure, chronic kidney disease, depression, and others.

“This helps to keep all medical conditions under control.” states Heather Toft, Clinic Director, “We are excited to be involved in this program, as we truly believe that our ability to care for your medical conditions will be extended beyond the clinic visits.”

Dedicated clinic staff will reach out to patients regularly by phone and help you set personal goals, learn more about your conditions, ask medical questions, appointment scheduling, medication review, and, most importantly, help patients get the right care at the right time.
