General Description:
Works collaboratively with providers, staff, and other healthcare professionals to provide comprehensive care management and care coordination across the health care continuum in order to improve outcomes and lower the cost of care. Assists provider and clinical staff with clerical/clinical functions for patients requiring chronic disease coordination. Acts as an integral member of the healthcare team who works to ensure safety, best practice, and high-quality standards of care are maintained. Works to ensure members are able to navigate through the healthcare continuum by improving the coordination of care and member/family experience. Coordinates a wide range of self-management support services as well as a wide range of community-based and health care support services for members. Works with providers, staff, and other resources to address specific quality improvement initiatives. Performs all functions of clinic nurse on a regular basis.
Experience/Education Requirements:
Graduation from an accredited program of nursing with current licensure as a Registered Nurse in the State of Iowa is mandatory. Bachelor’s degree in nursing or a related field preferred. Prior experience in nursing, physician clinic settings, care coordination, or similar preferred. Ability to adapt to flexible work schedule and frequent interruptions. Ability to prioritize to meet deadlines on daily work and special projects. Must be able to work independently with minimal supervision. Effective oral and written communication skills, excellent interpersonal skills. Must exhibit a high degree of responsibility for confidential matters.

General Description:
Performs a wide variety of duties relative to patient admitting, clerical support, supplies/inventory, etc. Responsible for answering and redirecting all incoming calls to the clinic to the appropriate contact. Checks in patients, schedules appointments, takes messages and greets patients/visitors.

Establishing and maintaining rapport with all individuals, both clinic and hospital personnel and the community at-large are essential. Works in an atmosphere that requires teamwork and an orientation toward maintaining a high public image.

Ability to accommodate a flexible work schedule during regular business hours.
Experience/Education Requirements:
A high school diploma, or equivalent, required. Relevant clinical experience preferred. Previous customer service experience required. Previous scheduling and reception experience in a healthcare setting, preferred. Must be able to multitask, good with computers, able to navigate multiple EHRs. Word and excel experience desired. Clinical experience as a nurse assistant or medical assistant desired.

General Description:
Audubon County Memorial Hospital and Clinics is seeking an Advanced Registered Nurse Practitioner to join our provider group in Audubon, Iowa. The emergency department is staffed full-time with nurse practitioners and 24/7 physician back-up via telemedicine.  This opportunity occasionally requires assisting in the rural health clinic with acute patient visits.
Experience/Education Requirements:
  • Graduate of an accredited ARNP program.
  • National certification as a Nurse Practitioner.
  • Unrestricted license in the state of Iowa as a Nurse Practitioner.
  • Active ACLS, ATLS, BLS, PALS required within 90 days of hire.
  • Credentialed as a provider with DEA number and prescriptive authority.
  • Prefer a minimum of two years’ experience in hospital and/or emergency medicine.
  • Able to communicate effectively with all members of the health care team.
  • Able to perform a variety of duties characterized by frequent change.
  • Working knowledge of computers and Electronic Health Records.

General Description:
Will perform general radiographic and CT procedures at a technical level not requiring constant supervision. Performs a variety of technical procedures requiring independent judgment. Takes responsibility for designated areas or procedures required. Manipulate, adjust and control the complex electronic and computerized components of the imaging system to produce patient examinations and research project while performing quality assurance procedures and evaluating system performance throughout the working day. This includes independent operation with minimal physician supervision. Works with all levels within the organization and maintains positive relationships, cooperation, communication and coordination of all activities.

 
Experience/Education Requirements:
Graduation from an accredited program of radiology with current registration through American Registry of Radiologic Technologists (ARRT) and permit to practice through the State of Iowa. Ability to adapt to flexible work schedule and frequent interruptions. Ability to prioritize to meet deadlines on daily work and special projects.
